Are you an ACHD patient or the partner of a patient with CHD? Share your story to show other patients or their partners that they’re not alone.

 

  • First, tell us about yourself. In the large text box, tell us your name, your CHD or your partner's CHD, your age, and where you're from. Next, select a topic from below that most relates to you. Then record a 5-10 min video introducing yourself and telling us your ACHD story as it relates to the topic you chose. Hold your camera or phone horizontally when recording. Be sure to check the box for the media waiver. If your submission has been selected, you will be notified by the ACHD Coordinator. Your video will then be used on the Conquering CHD website or media created by Conquering CHD.
